Private
Pay
Using a private pay option
allows access for therapy and counseling for any reason. No
diagnosable mental health condition is required for services.
Should you choose to use a private payment
option...

You
and your clinician decide what is most important to address,
without third party restrictions.

If
a diagnosis is made, it remains confidential and need not be sent
to another person or group.

You
and your clinician can determine the length and frequency of each
session.

Your
record and all information in it will remain between you and your
clinician, within the limits of the laws, regarding
confidentiality.

Relationship
and marriage concerns can be freely and specifically
addressed.

Your
out of pocket per session cost is likely to be higher than if
using a benefits package.

You
may have a Flex Plan to assist in payment through an
employer.
Using a Benefits
Package
Insurance benefits for
out-patient mental health are often included as part of a complete
medical insurance benefits plan. These benefits are yours and
are available for use in our office, if allowed by your
carrier. Should you choose to use your benefit
package...

Out
of pocket costs may be less expensive than a private pay
option.

A
diagnosable mental disorder is required. Your clinician will
assess your condition and make an appropriate diagnosis.

Your
primary care doctor or clinic may be contacted to coordinate your
medical and psychological treatment.

The
diagnosis and treatment plan becomes part of your permanent
medical record.

Your
insurance carrier may refuse payment for part or all of your
treatment, in which case you will be responsible for payment of
the difference.

The
number of sessions allowed may be limited by your insurance
carrier.

Should
your insurance carrier change, you may be unable to continue
therapy with the same clinician.

The
benefit package may limit having additional family members
participate.
-
Marriage
and relationship counseling is often not covered by benefits
packages.
Private Pay Fee

Life Dynamics, Inc.
offers several private payment options. These plans are
designed for people interested in enhancing, improving and/or
stabilizing their mental health and wish to have an increased level
of confidentiality regarding their treatment.
Session by Session
Plan
A Session by Session
plan provides the greatest degree of flexibility and opportunity for
a deep exploration of a person’s personality, the dynamics that are
at play within that personality, and there is no limit to the number
of sessions a person can use. This plan is discounted off our
regular fee to $100.00 per session given a minimum of 4 appointments
are part of the written contract. The first session will carry
an additional $40.00 intake fee.
4 or 6 Session
Plans
These plans are designed
for people who wish to do brief, focused work, usually around a
single issue. The sessions must be used within four months of
the first session.
4 sessions -
$360.00
6 sessions - $510.00
8 or 10 Session
Plans
These plans allow for
exploration and resolution of issues that are more complex.
All sessions must be used within six months of the first
session.
8 Sessions -
$640.00
10 Session - $750.00
Multiple Session Plan
Policies
- Multiple session plans
are paid in full at the conclusion of the first session by cash
and/or check.
- When a multiple
session plan is selected, the client and clinician establish a
written contract outlining the goals of counseling and the date
the final session is to be completed.
- No refunds are
provided for unused sessions.
- 24 hour notice is
required for canceling or rescheduling a previously scheduled
appointment.
- There is a $45.00
charge for appointments that were not previously cancelled.
- All sessions must be
used within the time limits stated in the written
contract.
